United Airlines Flight UA613 Experiences Critical Navigation System Malfunction
The United States National Transportation Safety Board (NTSB) has confirmed that United Airlines Flight UA613, which departed from Lagos to Washington in January, suffered a severe navigation system failure mid-flight.
Investigation Findings on United Airlines Flight UA613 Malfunction
According to the NTSB’s preliminary report, the aircraft’s autopilot disengaged due to the navigation system failure, leading to altitude fluctuations described as “altitude excursions.”
Key Data from the Report
- Approximately one hour and five minutes before the incident, flight data indicated a malfunction in the left Inertial Reference Unit (IRU).
- Around 55 minutes later, the right IRU also failed.
- At 00:30:57, the autopilot disengaged, forcing the flight crew to take manual control, which triggered both master caution and master warning alerts.
- At 00:31:14, a stick shaker activation occurred, indicating a possible stall warning.
- The autopilot was re-engaged briefly but disconnected automatically again.
- At 00:40:30, the aircraft initiated a right turn back toward Lagos.
- At 00:44:39, the autopilot was re-engaged and remained engaged until the final approach at Murtala Muhammed International Airport (LOS).
- The altitude instability lasted for approximately 12 minutes, reaching a maximum altitude of 36,000 feet.
Emergency Landing Back in Lagos
On January 23, United Airlines Flight UA613 declared an emergency while flying over Ivory Coast airspace and returned directly to Lagos. The aircraft, carrying 245 passengers, eight flight attendants, and three pilots, made a safe emergency landing at Murtala Muhammed International Airport (MMIA).
Possible Causes and Safety Concerns
Aviation experts suggest that dual IRU failure is a rare occurrence but can have significant implications on flight safety. The Federal Aviation Administration (FAA) and NTSB are further investigating the incident to prevent future occurrences of similar navigation failures.
